- Contact information for the complaining party, including name, address, email, phone number, and fax number;
- A statement that the complaining party believes in good faith that the material is being used without authorization;
- A statement that the provided information is accurate, and the complaining party is authorized to act on behalf of the copyright owner.
Section 17 USC §512(f) allows for civil penalties against those who knowingly misrepresent information in an infringement notification under 17 USC §512(c)(3).

Counter Notification – Restoration of Material

- Your name, address, telephone number, and consent to the jurisdiction of the federal district court.

Repeat Infringer Policy
